Job Title: Care Manger Department: Care Management Reports to: Care Manager Supervisor Summary Care Managers are responsible to coordinate the care of all individual’s enrolled in Health Home that are under their caseload. They play an important part in arranging the care of the individuals who are eligible for the Health Home program. A care manager is responsible for coordinating treatments for their clients including customized assessments and care plans based on their unique needs. A care manager builds relationships with their members and serves as their advocates. Care Manager connects their members with social and community support systems and educational resources. Care Managers collaborate with their member’s care teams to ensure quality care is provided to the member. Care managers evaluate the members progress and ensure care continuation. Care Managers are responsible to make sure their paperwork is compliant with CBC regulations and DOH requirements. Essential Duties and Responsibilities Procure required paperwork to ensure compliance Meet with individual and caregiver to engage with client and create rapport Administer CANS assessment Collaborate with care team and conduct IDT meeting Complete Safety and Emergency Plan, and Plan of Care Help member and care team access community resources Complete core services and necessary documentation as per CBC’S guidelines, including one face to face meeting per month Bill monthly once all necessary work has been completed Adhere to QA instructions and guidelines Attend training as necessary Qualifications Computer skills BA required Bilingual in English and Spanish Compassion and empathy towards individuals with special needs Ability to prioritize tasks and manage time effectively Professionalism and integrity in maintaining confidentiality and handling sensitive information Flexibility to adapt to changing requirements Excellent communication skills, both verbal and written, to interact with caregivers, care managers, service providers, etc. Attention to detail to effectively manage documentation and ensure regulatory compliance Note: This job description outlines the primary responsibilities of the Care Manager, but it is not an exhaustive list of all duties and tasks.
