Key Responsibilities Care Coordination & Client Support Serve as the primary point of contact for clients, families, and care team members Develop and monitor individualized care plans to ensure client needs are being met Address client concerns and escalate complex cases to the appropriate team members Scheduling & Operations Coordinate and manage caregiver schedules to ensure consistent coverage Track and follow up on authorizations, plan changes, and service updates Maintain accurate and up-to-date client records and documentation Communication & Collaboration Liaise between clients, families, caregivers, and internal teams to ensure seamless care delivery Provide timely updates to all parties on care plan changes and service status Support intake and onboarding of new clients as needed Qualifications Experience in a care coordination, case management, or homecare role Strong verbal and written communication skills Highly organized with excellent time management and follow-up abilities Compassionate, professional, and committed to client confidentiality Comfortable working in a fast-paced, in-office environment Interested?
