A Community Healthcare Facility is seeking a Full-Time Primary Care Physician to join their collaborative team. The candidate will play a central role in delivering excellent clinical care, with a strong emphasis on annual wellness visits, preventive screenings, and managing walk-in sick patients. The clinician will play a highly valuable role within our organization as a key member enforcing quality care following PCMH standards and reducing care gaps existing in NYC. Beyond the exam room, the ideal candidate is someone who will lead by example, foster a positive team culture, and work closely with ownership on strategic decisions and practice development. Responsibilities: Conduct comprehensive Annual Wellness Visits and preventive health screenings Diagnose and manage acute illnesses and walk-in patients in a fast-paced, community-focused setting. Maintain and grow a dedicated primary care panel, providing chronic disease management and care coordination. Collaborate with support staff to optimize clinical workflows and ensure excellent patient experiences. Act as a clinical leader—mentor staff, champion a culture of collaboration and accountability, and be a go-to for clinical decision-making. Maintain high standards of documentation, billing, and coding, ensuring compliance and revenue integrity. Partner with practice leadership to identify growth opportunities, improve operations, and implement care innovations. Maintain PCMH Quality Standard Improve Care Gaps in the patient population Qualifications: New York MD License 3+ years in primary care and/or family medicine Strong clinical skills with chronic conditions Comfortable prescribing controlled substances when needed Able to see both adults and children Open to growing with a busy practice Team player with good communication Culturally aware and respectful Salary: $290K+ Location: Brooklyn, NY
