Field RN – Home Care Location: Rockland & Orange Counties, NY Schedule: Full-Time Compensation: $55–$60/hour + Benefits A growing home care organization is seeking a Field RN to conduct in-home assessments and develop individualized plans of care for patients throughout Rockland and Orange Counties. This is not a skilled nursing position. The role focuses primarily on assessments, care planning, documentation, and coordinating appropriate services based on each patient's needs. Responsibilities: Conduct in-home patient assessments. Develop and update individualized plans of care. Evaluate patient needs and recommend appropriate services. Complete required documentation and follow-up. Communicate with patients, families, and care teams. Ensure care plans remain appropriate as patient needs change. Qualifications: Active New York State RN license required. Home care experience preferred. Strong assessment, documentation, and communication skills.
