Field RN – Home Care Location: Brooklyn, NY Schedule: Full-Time Compensation: $55–$60/hour + Benefits A growing home care organization is seeking a Field RN to conduct in-home assessments and develop individualized plans of care for patients throughout Brooklyn. This is not a skilled nursing position. The role focuses on patient assessments, care planning, documentation, and coordinating services based on patient needs. Responsibilities: Conduct in-home assessments and evaluate patient needs. Develop and update individualized plans of care. Coordinate appropriate services and follow up on patient needs. Maintain accurate clinical documentation. Communicate with patients, families, and care teams. Qualifications: Active New York State RN license required. Home care experience preferred. Strong assessment and documentation skills. Excellent communication and organizational abilities. Comfortable working independently in the field.
