LaSante Health Cente r is seeking a Social Care Network (SCN) Screener & Navigator to assist Medicaid members through the Social Care Network process. The ideal candidate is organized, compassionate, detail-oriented, and comfortable working with clients by phone and navigating online systems. Responsibilities: Contact clients and confirm demographic and household information Obtain and document required consent Verify Medicaid eligibility and coverage Conduct Social Care Network screenings and eligibility assessments Identify eligible social care services and assist with referrals Create and manage cases within the SCN platform Follow up with clients and document all outreach and case activity Maintain accurate, timely, and compliant records Coordinate with providers and other team members to ensure clients successfully receive services Qualifications: Strong communication and computer skills Excellent attention to detail The ability to manage multiple cases efficiently.
